Free headache diary (printable PDF)

A diary helps you and your doctor see how often headaches happen, how bad they get and what might come before them. Print it, keep it by your bed, and fill it in each evening. No email needed.

Attack log

One row per attack, with more detail: start and end time, where it hurts, symptoms and whether your medicine helped.

How to fill it in

  1. At the top, write your name, the month and any medicines you take every day.
  2. Each evening, fill in the day's row. In Headache today? write Y or N.
  3. If you had one, give it a number in Pain 0–10. The pain scale at the top helps you keep the numbers the same from day to day.
  4. Under Possible triggers, note anything that came before it, like poor sleep, stress or a skipped meal. In Medicine, dose and time, write what you took, how much and when.
  5. At the end of the month, count your headache days, the days you took medicine and your worst day, and fill in the box at the bottom.

Using the attack log? Add the start and end time, shade where it hurts on the two heads, and give your medicine a score in Did it help? 0–5.

Bring it to your appointment

Take the diary with you. It helps your doctor see:

  • how many headache days you had each month
  • your worst days, and what came before them
  • what you took, how often, and whether it helped
